Portal Registration Form:
To protect the privacy and security of your health information- If SRMC does not have an email address on file that matches your enrollment request, you will NOT be able to enroll until that information is provided.

  • Print the Portal Registration Form using the link provided, or call 330-332-7536 to get one mailed to you. Then bring the completed form to SRMC's Medical Records Department with your photo ID.
  • Or, you can provide an email address to SRMC's Registration staff during your next hospital visit.
