How To Enroll For The
SRMC Patient Portal & Online Bill Pay

Do we have your email on file?

- If “Yes,” and you provided your email after 1/12/21, a Patient Portal account has automatically been created for you and you should have already received an email invitation to complete the enrollment process and establish your log on ID and password.

- If  “Yes,” and you provided your email before 1/12/21 and have not yet enrolled in the Patient Portal, please “skip” to the SELF-ENROLLMENT instructions* below.

- If “No,” please click here to download and print the “Portal Enrollment Form.”

  1. Complete the enrollment form.
  2. Scan or take a photo of the completed form, along with your photo ID. Email both items to PortalEnrollment@salemregional.com. Please include your desired email address that you want used within the body of your message.
  3. You will receive a return email within 1-2business days about the status of your Portal enrollment, along with a temporary logon and password, which is needed to set up your account.

* SELF-ENROLLMENT for Patient Portal & Online Bill Pay

If you provided your email to SRMC staff before 1/12/21 and have not yet enrolled in the Patient Portal:

  1. Using the email address you provided to SRMC staff, complete the online "Self Enrollment" page by clicking here.
  2. Shortly after you complete the self enrollment process, you will receive an email with a temporary password and logon to set up your account.
  3. Once your account has been set-up, click here to access SRMC's Patient Portal and Online Bill Pay.

To protect the privacy and security of your health information: If SRMC does not have an email address on file that matches your enrollment request, you will NOT be able to enroll until that information is provided.

Portal Registration Form:

  • Print the Portal Registration Form using the link provided, or call 330-332-7536 to get one mailed to you. Then bring the completed form to SRMC's Medical Records Department with your photo ID.
  • Or, you can provide an email address to SRMC's Registration staff during your next hospital visit.
